Health forms needed for schools such as sports physical forms, medication authorization form, registration health forms, emergency information and general guidelines.


  • Medication Administration Guidelines
    The intent of these guidelines is to assure safe administration of medications for those students who require them. Local school district policies must be established that assure this process.(English) (Spanish)

 

  • Medication Administration Form
    The Medication Authorization form is necessary for any medications that are to be taken at school or during the school day. Forms must be signed by parent. The school must have a form on file for any medication (even over-the-counter) to be at school. (English) (Spanish)

 

  • Authorization to Disclose Health Information Form
    This form of Authorization to Disclose Health Information allows you the flexibility to determine what types of information are to be released and under what circumstances. In addition, this form complies with the HIPAA (Health Insurance Portability and Accountability Act) Privacy Rules. (English) (Spanish)

 

  • Athletic Participation Physical Evaluation Form

  • Medical Questionnaire
    This form is to be filled out by the parent or guardian and given to the nurse when a student starts school to provide information regarding the student's specific medical condition.
